As Alzheimer’s disease progresses, a person’s needs change and it is important to recognize and address those changes with appropriate care. Along with meeting basic needs, experts say care providers should treat the whole person by creating an environment that allows them to feel independent and safe. The Alzheimer’s Association says determining the best care options can be tough and offers some advice on how to ensure the person has the services, programs, and caregivers in place for the best possible quality of life. 

The first step to any decision regarding care is to honestly evaluate your loved one’s needs. Also, to whatever degree possible, it is recommended you include them in any decisions. Another thing to remember is during the early stages of the disease the person may be able to live independently but in the middle stages, 24-hour supervision is needed. Additionally, around-the-clock care is much more demanding in the later and end stages.    

Primary points to consider include safety especially in terms of unsupervised cooking, or if the person has a tenacity to wander or needs help with medications. Does the person have medical conditions that need monitoring such as diabetes or a mobility issue? Other problems that may demand more professional care include a person becoming aggressive or needing help with the ADLs (activities of daily living). Learn More About Omega-3 Fatty Acids

While we always hear about omega-3 fatty acids and how they are good for us, just what are they? The experts explain omega-3s are a type of polyunsaturated fat also called “healthy fats.” Research suggests they play a role in protecting us from heart disease and dementia. DHA (docosahexaenoic acid) and EPA (eicosapentaenoic acid) omega-3s are “the primary polyunsaturated fats in brain cell membranes and have been popularized and successfully marketed as dietary supplements.”  

DHA, EPA and DPA omega-3 fatty acids are synthesized by marine organisms such as algae and phytoplankton. They are then consumed by fish, aquatic mammals and crustaceans where the acids are stored in body fat, liver and blubber. When we consume foods from the sea such as salmon, tuna, sardines, mackerel, liver of lean white fish and fish oils, we get the benefits of the omega-3s stored in their bodies.  

Another type of omega-3 is ALA derived from plant sources and is the most readily used by the body to create all other types of omega-3 fatty acids. ALAs are found in nuts and seeds like flaxseed, chia seeds, and walnuts as well as oils like canola and soybean. Experts say although ALA can create all forms of omega-3 fatty acids, “the conversion of ALA to DHA and EPA is poor, so they recommend a dietary intake of a combination of foods rich in ALA, EPA, and DHA.

Along with reducing the risk for heart disease, The Medical News Today (MNT) reports other evidence-based benefits of omega-3s include exerting anti-inflammatory effects throughout the human body, lowering blood pressure and cholesterol levels, and improving tolerance to cancer treatments. A study also showed fish oil supplementations protected against major depressive disorders in persons between the ages of 15 to 25.

As people age, many find their appetites have diminished or they suffer from physical or medical conditions that make eating more difficult.  Therefore, the nutritional value found in the foods they do consume is important, and even today, many seniors have an “inadequate intake of vitamins A, B, C, D, and E, folic acid, calcium, and niacin.”   

The U.S. Department of Agriculture (USDA) and the Department of Health and Human Services (HHS) advise that people over the age of 50 follow the USDA food recommendations, which should help keep older adults feeling more vital, and thus healthier.  They also point out the earlier you start eating a nutrient-rich diet, the more effective it will be later down the road.  Recommendations include a daily diet of fruits, vegetables, grains, protein foods, and dairy.  It also includes eating foods with healthy fats such as olives, nuts, and avocados.

Activities to Improve Cognitive Abilities

As we grow older, not only do our physical abilities slow down but so do our cognitive abilities. We may find ourselves not able to remember facts the same way we used to or have difficulty thinking about two problems at the same time. Professionals say these changes make it harder for us to stay focused and easier to get distracted. You may not recall new information as you did in the past because you didn’t really learn it. 

Changes can start as early as in our 50s and 60s, but experts say to remember that most age-related memory loss and thinking problems are normal and not necessarily a sign of Alzheimer’s disease. They explain it can “simply reflect a slower processing speed and poor encoding and retrieval of new memories as a result of diminished attention.”  Despite your brain moving a little slower, your judgement is still intact, and you can make sense of what you know and form reasonable arguments. Another thing to consider is even the smallest hearing loss can make processing new information more difficult. 

Research shows while our ability to concentrate diminishes, there are things we can do to improve it.  A 2015 study found participants who played brain games 15 minutes, 5 days a week had a big impact on improving their concentration. Another small study in 2018 saw an improvement in seniors’ visual selective attention (VSA), the ability to concentrate on a specific task while ignoring distractions, after playing video games. Recommended cognitive activities include things like crosswords puzzles, Sudoku, chess, jigsaw puzzles and other memory games.

To help, they offer easy tips on how to painlessly replace calorie-laden foods with healthier, lower-calorie substitutes. In addition to the fact that nutrient-rich foods appear to help individuals be successful in their quest to lose weight, meals do not necessarily have to be smaller than your average intake. 

Dietitians suggest an easy food swap is to drink water flavored with lemon slices or sparkling water rather than sugary sodas. One study found by replacing refined grains with whole grains it “improved blood cholesterol levels and decreased percent of body fat in postmenopausal women.” Replacing sugary granola with heart-healthy oatmeal can also help control your weight. Not only is oatmeal a good source of soluble fiber known to reduce cholesterol but it contains magnesium and potassium known to promote healthy blood pressure. Also dietitians suggest trying sprouted grain breads, which many find tastier and healthier than even whole-grain breads.

Other ways to get healthier and lose weight is to include more plant-based foods into your diet. Make a pledge to have a couple of meatless dinners each week and get your protein from bean burgers, lentils, and quinoa instead of hamburgers and steaks. To get more veggies into your diet, top your bean burger with spinach or lettuce, tomato slices and mushrooms instead of cheese. Rather than pasta, use squash or zucchini noodles.

Even those with a Sedentary Lifestyle may be Affected by Heat Stroke

Doctors warn the elderly as well as those who care for them should be aware of the dangers of heat-related illnesses. They stress that you do not have to be a marathon runner to suffer from heat stroke. People young and old are at risk for serious complications from heat-related illness. In 2015, the latest year for which such statistics are available, the CDC reports heat stroke claimed 335 lives in the United States. 

Experts explain there are two kind of heat stroke: “exertional, which is associated with vigorous physical activity, and non-exertional.” The elderly or people with chronic health conditions or suffering from obesity are more susceptible to non-exertional heat stroke. The same goes for individuals on blood pressure lowering drugs like beta blockers and diuretics; or taking clozapine, a drug used to treat psychiatric disorders. Drinking alcohol can cause dehydration and also put you at a higher risk for heat-related illness. 

Signs of heat-related illness include fainting, behavior changes, high body temperature (over 104° F), dry skin, a strong and rapid pulse and a slow and weak pulse. The person may no longer be sweating even though it is hot.  

To lessen your risk for heat-related illness, doctors recommend avoiding being outside during the hottest part of the day, staying hydrated and drinking lots of water even if you do not feel thirsty. The National Institute of Health says during a heat wave, if you do not have air conditioning, take a cool shower and wear loose fitting clothes. If you need, take an air conditioning break by going to a mall or an air conditioned community center to cool down.

Regular Physical Activity Can Reduce the Risk of Falls in Older Adults

Not only is physical exercise good for our heart and cognitive health but it can help older adults stay steady on their feet. Studies show regular physical activity can reduce the risk of falls in older adults at a high risk of falling by nearly one third. 

Experts explain certain kinds of exercise helps increase muscle mass and bone strength as well as improve balance and coordination, fight off osteoporosis, and reduce the risk for fractures and broken bones. Some of the best exercises are known as weight-bearing, which require a person to carry their weight and work against gravity. Examples include walking, dancing, hiking, climbing stairs, jogging and playing tennis or basketball. Doctors say if you can manage it, increasing your pace during aerobics, jogging and power walking make the exercises even more effective at strengthening bone density.  

Resistance exercises also work against gravity and help build muscle mass and improve bone strength. Strength training with weights, weight machines or resistance bands are all examples of exercises that rely on muscle contractions that tug on bones to stimulate them to bulk up. Along the same line, impact exercises such as jumping jacks and walking or running uphill have been shown to improve bone density fairly quickly. 

Healthcare providers say to be sure to check with your doctor before starting any new exercise program, especially if you're taking medications that slow your coordination or throw off your balance. According to the Surgeon General, your optimal goal should be at least 30 minutes of physical activity on most days, preferably daily.

The various stages of grief can include shock, anger, disbelief, guilt, and profound sadness. Grief can adversely impact our physical and mental health and may interfere with many aspects of our life including sleeping and eating. Another thing experts say is to keep in mind grief is a personal experience and everyone goes through it in their own way and in their own time.   

If you are a person whose grief has become overwhelming, it may help to share your feelings with a grief support group or to talk to a professional grief counselor. Myths surrounding grief include you must stay strong, or you need to ignore it, and it will go away. Professionals say for real healing, people need to face their grief. Another myth regards the timeline. Many people think the grieving process should take a year, but for many it may take longer. Moreover it is important to remember accepting the loss does not mean the person is forgotten.   

To grieve, acknowledge your pain and accept the fact you can feel a range of emotions including anger at the situation or at the person for “deserting you.” Treat yourself with love and support, and see your doctor if you continue to have a hard time.

August is National Eye Exam Month

Optometrists and ophthalmologists explain practicing good eye care includes regular eye exams. The current recommendation for those between 40 to 65 years of age is an exam every 2 to 4 years. For adults over 65, the recommendation is every 1 to 2 years and should include a check for eye diseases and conditions such as cataracts, age-related macular degeneration, glaucoma and diabetic retinopathy.

During a dilated eye exam, an ophthalmologist will check pupil reaction, eye pressure, retina, optic nerve, side/peripheral vision, eye movement, muscle balance and coordination. They will also check your focusing ability and eyeglass prescription. The doctor will ask about your medical history including if you or a family member has diabetes, as diabetics have a higher risk of developing cataracts and glaucoma as well as diabetic retinopathy. In addition, they will want to know if you have high blood pressure as over time it can cause damage to the blood vessels around the eyes. Shingles is another disease that can cause inflammation around the eyes, swelling of the eyelids and pain.

Tips for Safely taking Prescription Drugs

Every year seniors are hospitalized due to prescribed medications. Statistics show drug-related problems are common in older adults and include drug ineffectiveness, adverse drug effects, over-dosage, under-dosage, inappropriate treatment, inadequate monitoring, nonadherence, and drug interactions. It is important to remember even though medication is prescribed by your healthcare provider, problems can occur.  

Experts say you must follow directions and take medications as prescribed including on time, in the right dosage, with or without food, and never grinding it into a powder (unless directed by the doctor). They recommend carrying an updated list of all medications and sharing it with your doctors. Additionally, be aware of common interactions between prescribed drugs and over-the-counter medications and alcoholic drinks. 

Before your doctor prescribes a new medication, ask them why you are taking it, for how long and what are the potential side effects. It is important to know what to look for and to discuss the benefits versus any negative or dangerous consequences. You will want to know things like will a drug make you drowsy so you know not to drive under its influence.  

Another thing to safeguard your health is to review your medications with your doctor on a regular basis asking questions such as: do you still need to take it, is the dosage correct, and is there a non-drug alternative? Despite your faith in your medical team, it is not a bad idea to check your medications against the American Geriatric Society’s “2019 American Geriatrics Society Beers Criteria for Potentially Inappropriate Medication Use in Older Adults.” The online site lists medications that may be inappropriate or dangerous for seniors.

How to Talk to Your Kids about Alzheimer’s Disease

When a grandparent or another close family member or friend suffers from Alzheimer’s, it can be difficult for children to understand especially if they are very close to the person.  Most likely they have questions and concerns regarding the disease and may be struggling with a range of emotions including sadness and confusion. They may not know how to act around the person anymore or be frightened by the “strange behavior.”

Having grandma or grandpa call you by mom or dad’s name or asking the same question again and again are just some of the strange things kids see happening to a beloved grandparent. They may worry Alzheimer’s is contagious and it is important they know it isn’t but rather a degenerative disease that happens in the brain. If the person lives with them, teenagers may be embarrassed to have friends around due to the person’s behavior, and sometimes children withdraw from the person or lose patience.  

The Alzheimer’s Association suggests sitting down with your child and telling them while the person may forget your name, they can still feel your love. Encourage questions and provide honest answers, and perhaps together you can learn more about the disease. The experts offer ideas on things kids can do and consider this as alzheimers care with the person with alzheimers as listening to music, setting the table, working on a puzzle, reading a book or looking at family photos. As the person’s Alzheimer’s disease progresses, make sure your kids feel comfortable asking more questions. Regardless of how old we are, losing a loved one to Alzheimer’s disease is challenging for everyone.

Commonsense Precautions to Help Prevent Another Fall

Many of us have heard about or know a senior who has had a fall that leads to a serious injury. Each year statistics report one out of every five falls causes an injury, such as broken bones or a head injury; and approximately 3 million older adults are treated in emergency departments for a fall injury. Annually, falls result in 32,000 senior deaths. 

The Harvard Medical School reports the highest risk factor for a fall is a previous fall. While a senior may have fallen or tripped and not thought much about it, as they only twisted an ankle or didn’t sustain a serious injury, the event may be a precursor to another fall. 

There are things seniors can do to protect themselves from a fall and it starts by being aware of any risk factors you may have including vision and hearing problems, which can make a fall more likely. Experts say stiff joints can hamper your gait and balance and make it easier to fall, as do certain medications for blood pressure and antihistamines. As a deficiency in vitamin D decreases a person’s bone mineral density and strength of the bones, it too can cause instability and raise the risk for a fall. Experts report even something as unrelated as a head cold or the flu can cause dizziness and increase your risk.  

To help boost core strength and improve balance, talk to your healthcare provider about the best type of exercises to do. Other steps to decrease the likelihood of a fall are to keep moving to loosen stiff joints and help build strength, wear sensible shoes and remove home hazards. Finally make sure your home has plenty of good lighting.

Dementia may be Confused with Depression

Experts report as people age, dementia and depression are both common conditions and may be confused with one another. Additionally, one may lead to the other as depression is linked to a higher risk for dementia, and dementia can cause depression. 

The Harvard Medical School explains depression in the elderly can lead to a “phenomenon called pseudodementia—an apparent intellectual decline that stems from a lack of energy or effort.” They say individuals with this problem are often forgetful, move slowly, exhibit low motivation and signs of slowing mental prowess. If a loved one experiences these types of symptoms, their doctor may treat it with the same medications used to treat depression. As the person’s mood improves, they typically begin to function at their previous level. 

Professionals share a few ways depression and dementia differs to help families and healthcare providers determine what is going on. One important difference is the time span of the decline.  A person with depression typically shows a faster decline in mental functioning than those suffering from Alzheimer’s or other dementia-related diseases. Also, Alzheimer’s disease causes confusion and disorientation as well as problems with short-term memory. Depression causes problems with concentration. 

People suffering from depression do not have more than the usual difficulties with writing, speaking and motor skills, and are more aware of any memory problems they are experiencing. People suffering from Alzheimer’s disease seem indifferent to those types of changes. However, to determine the cause of the problem, it is important to make an appointment with the person’s healthcare provider.

Loneliness is Linked to Poor Health

According to the Centers for Disease Control & Prevention (CDC), statistics show seniors who report feelings of loneliness are at higher risk for several diseases including dementia and are more likely to experience premature death. The National Academies of Sciences, Engineering, and Medicine (NASEM) says “one-third of adults aged 45 and older feel lonely, and nearly one-fourth of adults aged 65 and older are considered to be socially isolated.” Social isolation is a lack of social connections and a result of a person living alone, having a spouse and/or other supportive friends or relatives die, or suffering from chronic illness. Other related problems include a lack of transportation, mobility impairment and untreated hearing loss, which all limit the senior’s opportunity to engage with others. 

AARP Research surveyed adults 45 and older regarding their feelings of loneliness. Of those respondents who rated their health as excellent, only 25 percent said they were lonely. Among participants who said their health was poor, 55 percent reported feeling lonely. The CDC also says studies find social isolation or loneliness is linked to a 29% increased risk of heart disease and a 32% increased risk of stroke as well as a 50% percent increased risk of dementia.

Studies Show a Healthy Diet May Help Reduce the Risk of Cognitive Decline

Researchers have found evidence suggesting genetics, environment, and lifestyle can affect our risk of developing Alzheimer’s disease and dementia. A poor diet is one of those lifestyle choices that causes high blood pressure and heart disease and increases a person’s risk for Alzheimer’s disease. 

WebMD.com reports the MIND diet; a combination of the Mediterranean and DASH diets has shown promise in reducing the risk or slowing cognitive decline. They cite early studies showing MIND lowered the “risk of Alzheimer's by 53% in those who follow it closely and by 35% in those who follow more loosely.” In 2018, the MIND diet recommendation was expanded to include older adults who have suffered a stroke. 

To adhere to the diet, weekly meals should include food from the following groups: green leafy vegetables, other vegetables, nuts, berries, beans, whole grains, fish, poultry, olive oil, and wine. They say a good way to start is with a daily salad, one other vegetable, and three servings of whole grains every day. In terms of vegetables, collard greens, kale, and spinach have been found to be especially healthy. 

Poultry should be eaten once a week, fish twice a week and beans every other day. Nuts and berries make good snacks, blueberries and strawberries being some of the best. Olive oil not only adds a great taste to foods but has been shown to improve brain function over the long term and protect against dementia.
