Help Prevent Falls with Balance Exercises

The Centers for Disease Control and Prevention’s latest statistics show that one in four Americans aged 65 years and older falls each year.  Falls are the leading cause of fatal and nonfatal injuries among older adults, and the number of falls will continue to increase as America’s baby boomers grow older.

While these are worrisome facts, experts say there are some simple exercises seniors can do to help prevent falls and maintain their independence. Remember a common injury caused by an elderly person falling is a hip fracture that can seriously impact their life.  

Most balance exercises are easy to learn and do, do not require special equipment or a trip to the gym, and can be done anywhere.  The Mayo Clinic explains that while many exercises including walking can help maintain good balance, there are specific exercises targeted at improving a senior’s balance and stability.  They do caution however that if you have severe balance problems or an orthopedic condition, to get your doctor's okay before starting balance exercises.

Examples of balance exercises include: standing on one foot, walking heel to toe, back leg raises, and side leg raises.  To do the standing on one foot exercise, stand behind a sturdy chair; gripping for balance, lift one foot off the floor.  Hold for 10 seconds and then repeat with the opposite leg.  Repeat the exercise several times and increase the repetition as your strength increases.   

Walking heel to toe is done by positioning the heel of one foot in front of the toes of the other foot.  Your heel and toes should be touching.  Now select a spot ahead of you and focus on it to help keep you steady as you walk.  Try to step forward for as many as 20 steps.  Be sure to ask your healthcare provider for other balance exercises.

One suggestion is to buy a loved one a fitness watch. The marketplace has a wide assortment of watches as well as sensor devices to measure your daily footsteps and help keep you moving. Research shows measuring your daily activity improves your fitness level and can help people with obesity, diabetes, and heart disease boost physical activity levels.  JAMA Network Open published a finding which reviewed 38 randomized clinical trials with 4,203 participants.  They stated that “interventions with wearable fitness trackers were associated with significantly increased physical activity levels after approximately 15 weeks. Devices such as pedometers or trackers that count steps were associated with greater levels of physical activity in about 70 percent of the studies examined.”

Another healthy gift idea is a massage gun. Known as a self-myofascial release tool or SMR, they help overworked muscles recover and heal. Foam rollers additionally provide much of the same benefit at a much lower price point. However, experts caution before treating an injury or other medical problems including bone fractures, blood clotting issues, sensory deficits or wounds you need to consult with a physician or licensed physical therapist. 

Other healthy gift ideas include an electric kettle with a selection of antioxidants teas. Help Lower Your Blood Pressure with Lifestyle Changes

According to the CDC, in 2017 the American College of Cardiology and the American Heart Association published new guidelines for hypertension management defining high hypertension as blood pressure at or above 130/80 mmHg. Stage 2 hypertension was defined as a blood pressure at or above 140/90 mmHg. Nearly half of all adults in the US have hypertension or are taking medication to lower their numbers. Hypertension puts you at risk for heart disease and stroke, which are the leading causes of death in the US.  

If you have been told you have HBP, the Mayo Clinic recommends several lifestyle changes to help lower your numbers and to keep them down. They say if you are overweight you lose weight as blood pressure often increases as your weight goes up. Moreover, being overweight can result in sleep apnea that often raises BP. Even your waistline can put you at risk. Typically a waistline of more than 40" for men and 35” for women can be problematic but consult your doctor as there are some variations among ethnic groups. 

Regular exercise is another component to lowering and maintaining blood pressure. Experts recommend 150 minutes a week or 30 minutes on most days as studies show this amount of physical activity can lower BP by about 5 to 8 mm Hg. 

A healthy diet also plays a role in maintaining a healthy BP. Dieticians say read labels, eat plenty of whole grains, fruits, vegetables and low-fat dairy products. Avoid foods high in saturated fat and cholesterol; and reduce sodium by eating less processed foods and reading food labels. Also don’t add salt to your food. Talk to your doctor about other guidelines for lowering HPB.

Habits That are Bad for Brain Health

Our modern-day lifestyle encourages behaviors that are bad for the health of our brain as well as other parts of our body. Research finds the following four habits are especially dangerous for cognitive health. They are sitting, lack of socializing, inadequate sleep, and chronic stress. The good news is they are habits we can easily change. 

Many of our workplaces and entertainment pursuits include sitting for long periods of time. In fact, a 2018 study published in the PLOS One reports people aged 45 to 75 sit for an average of 6½ hours a day. Researchers found “sitting too much is linked to changes in a section of the brain essential to memory.” Using MRI scan, they looked at the region of the brain known to make new memories called the medial temporal lobe (MTL). Comparing the number of hours a day the person spent sitting, those who sat the most had the thinnest MTL regions, a precursor to cognitive decline and dementia. To help improve the functioning of your MTL, the study’s authors recommend getting up and moving at least every 30 minutes. Your activity could be a quick round of squats or pushups or a walk around your front yard, or somethings similar for a few minutes. 

Lack of socialization can also lead to a thinning of the MTL as a number of studies link loneliness to a higher risk of dementia. Experts say you don’t have to completely change your lifestyle but rather try to routinely connect with three or four family members or friends. The number of social connections is less important than the degree of meaningful interactions.  

Other healthy cognitive behaviors include getting at least seven to eight hours of sleep each night and reducing your level of stress. Research shows chronic stress “can kill brain cells and shrink the prefrontal cortex, the area responsible for memory and learning.” To manage stress, experts suggest breathing techniques and practicing mindfulness.

Learn More About Mild Cognitive Impairment

Studies report approximately 12-18% of people age 60 or older have some level of Mild Cognitive Impairment (MCI). So how do you know if you are one of them? The first step to diagnosing MCI typically comes after a person starts to complain of forgetfulness or cognitive problems, or initiated at the urging of family members. Doctors explain it is usually more serious if it comes at the request of those closest to the person because as a person develops more of the signs of Alzheimer’s disease, they are often not aware of their memory loss. 

Your doctor will begin with a thorough evaluation to try and determine if the problem is MCI, Alzheimer’s disease, normal age-related memory loss, or caused by another factor. It is important to note not all cases of MCI develop into dementia and an evaluation could identify medications as the problem. A Harvard Medical School article states, “Establishing the cause and scope of the impairment is helpful in setting a baseline for future evaluations to determine if the condition is progressing to dementia.” 

Some of the preliminary tests include a complete blood workup to rule out anemia, infection, kidney and liver disorders, or diabetes. Other tests may check for thyroid abnormalities, elevated blood calcium and vitamin B12 deficiencies. 

Cognitive tests begin with an in-depth medical history as well as questions regarding the person’s memory lapses and other thinking problems. Your physician will probably request a family member be there to reaffirm and document the changes. 

Your physician may refer you to a neuropsychologist for more tests including paper and pen and computer tests to assess memory, language and other cognitive skills including motor and visual function. If deemed necessary, they may do structural and functional brain scans or an amyloid PET scan, which detects in a living brain the signs of amyloid plaques, the hallmark of neurodegenerative diseases like Alzheimer's.

A Diagnosis of Alzheimer’s Disease is Difficult for Everyone

A diagnosis of Alzheimer’s disease is not only difficult for the person but affects their spouse, children, other family members and anyone who cares for them. However, experts say it is important to remember the person who is most affected, and if they ask you what is wrong, you should be honest and simply tell them. It may even give the person a sense of relief to know they are not insane and there is an explanation as to what is happening to them. 

That being said, medical professionals recommend if you can, have the person’s physician tell them but remember, a person afflicted with Alzheimer’s disease is forgetful, especially regarding new information. This means they will probably ask again (and again) what is wrong with them and it will be up to you to tell them. Doctors advise you to give a brief explanation.

Additionally, the rest of the family needs to be told as well as the person’s friends. Doctors say often he or she will appear to be healthy especially in the early stages so it is only fair to let others know so they do not mistake “odd” behaviors for craziness or meanness. 

Another reason it is helpful for people to know about the person’s condition is they can do some research on how to best respond and communicate with their loved one or friend. The National Institute on Aging (NIA) says to:

Give simple, step-by-step instructions.

Repeat instructions and allow more time for a response. Try not to interrupt.

Not talk about the person as if he or she isn't there.

Not talk to the person using “baby talk” or a “baby voice.”

Downsizing Can Be the Start of a New Adventure

Instead of feeling sad about moving from a home that perhaps is too large or costly for you to care for, think about moving into a smaller apartment or condo as the start of a new adventure.   However, to do it right, you should be organized.  Experts suggest you only take with you the things that bring joy and find ways to get rid of the rest.  If that piece of advice still finds you with way too much stuff, then take a moment to picture how you want your new life to look.  Is it a sleek, minimalist décor or more like a smaller, compact version of your present home?

Some simple solutions to help you decorate your new apartment include bringing with you or buying multi-purpose furniture.  This could include upholstered storage benches or cubes, nesting coffee tables, extension desks, and sized-down sofa and chairs more suited to a compact footprint.  Attractive baskets and storage carts also help to make a small place feel more organized.  

If you want to use some of your current furniture, measure it, and then draw up a floor plan of the new place and lay it all out to see how it will fit.  In addition, since your new place is smaller, it will most likely not have as many closets as your old home so go through your clothing and only take the items you truly enjoy wearing.  Give the rest to charity.  Lastly, consider the amenities at your new place.  For example if they have an onsite fitness room, you probably do not need to bring along your own in-home treadmill or stationary bike.

Alzheimer’s causes More Changes than Just Memory Loss

Along with changes in the person’s memory and cognitive abilities, Alzheimer’s affects a person’s physical abilities. Experts explain dementia symptoms can change everything from the way a person walks, talks, and how their body works. Normal body functions such as going to the bathroom, carrying on a conversation and maintaining balance become more and more difficult.  

While unfortunately we do not yet know the cause of Alzheimer’s, we do know the symptoms are caused by a buildup of harmful proteins in the brain called amyloid and tau, which create clumps called tangles and plaque. These clumps get in the way of normal brain functions and kill healthy cells.

Changes caused by Alzheimer’s include loss of balance and coordination, which puts a person at a higher risk of a fall. A person’s feet may shuffle or drag along as they walk. Another symptom includes weakened muscles and fatigue that makes it difficult to stand or sit upright in a chair. Trouble controlling the bladder and bowels as well as seizures and twitches are other common symptoms.    

While some people experience a slower pace of the disease and live up to 20 years after a diagnosis, the average life expectancy is 4 to 8 years. The most common cause of death in a person with Alzheimer’s is pneumonia, anothe infection, or a heart attack.

Gardening can be a Healthy, Enjoyable Hobby for Seniors

Research shows gardening may provide seniors both mental and physical benefits. In addition to enjoying a garden filled with beautiful flowers or homegrown vegetables and herbs, gardening can be an inspirational “mind-body-spirit” experience.  

Experts say before a senior picks up a shovel, it is a good idea to prepare in advance and take a few precautions. Gardening can put a lot of stress on the spine so it is recommended you warm up your body with a few, full body stretches before you start; during your garden session and after you are done. Use garden tools that are sharp and ergonomically-designed to cause less stress to the hands and back. Terraced or raised beds limit the amount of bending and stress on the back. Use a bench or knee pads as much as possible. Take precautions when lifting and limit your load. Use a light-weight wheelbarrow and keep your gardening sessions short.

Remember to stay hydrated and avoid the full heat of the day. Wear sunblock, protective clothing and a hat with a wide brim. Working in the dirt is therapeutic, and studies show the bacteria in dirt are actually good for us as they boost our immune systems.  

Several studies report gardening can provide some protection against dementia as well as slow the progression of dementia; and has the added benefit of being easy to do even when other activities become difficult. Feeling the dirt between your fingers as you tend to your flowers and vegetables is mentally uplifting, and it is highly satisfying to behold the results. There is even a social component when gardening in a community garden or being in your front yard and exchanging pleasantries and gardening tips as neighbors pass by.

Ask Questions before Getting Discharged from the Hospital

Experts say it is important to start thinking ahead to your recovery as you begin to heal from an injury, surgical procedure or illness. This includes proactively thinking about questions you should ask before you are discharged from the hospital. Dr. Ruby Sahoo, a member of the Society of Hospital Medicine’s board of directors and the hospital medicine performance director at TeamHealth in Austin, says there is no need to be shy about asking questions as doctors and nurses welcome them. 

Dr. Sahoo explains even though the hospital discharge team will go over much of the information you need to know, your questions can give you a better understanding of your diagnosis and treatment, and a better understanding of how to help yourself heal. This is important as a 2021 report found on average, 14% of people were readmitted to the hospital within 30 days of their initial stay.  

The questions you ask regarding your healthcare can also alert doctors to specific issues such as a patient remembering they previously took a medication now being prescribed at an earlier time and had a bad reaction. Knowing this your team can find a better alternative. 

To aid your healing process and help prevent the need to be readmitted, before leaving the hospital make sure you know the medications, the dosage, how to take them and why you are taking them. Ask if doctors found any other problems during your stay?  Also what type of follow-up appointments will you need, and has your primary doctor been updated? Additionally, what symptoms indicate you need to seek medical attention or return to the hospital?

As we age part of our baggage includes collecting an extensive medical history. However much our past history may be linked to our health today, it doesn’t necessarily follow us around. Experts explain we have yet to develop an all-knowing system that tracks a person from birth. Kelly Batista, executive director of the Brian D. Jellison Cancer Institute at Sarasota Memorial Health Care System in Florida, says for some, compiling a thorough medical history can become overwhelming.

Medical professionals recommend a couple of methods to help keep track of vital medical information. The first is to use your patient portal attached to your healthcare provider or hospital. Along with storing medical history and test results, you can use it to communicate with your doctor, ask questions, and confirm and request appointments. It allows you to access information regarding prescribed medications and to request refills, view doctors’ notes after a visit, update insurance and make payments for care. Although remember unless you are part of a large medical care system, your records are not all linked.

Mobile apps are another method to store medical information and are designed to keep track of diet, fitness, chronic health conditions like diabetes and day-to-day health events. Rema Padman, a healthcare informatics, analytics and operations researcher and Trustees Professor of Management Science and Healthcare Informatics at Carnegie Mellon University, says, “We’re also seeing apps and wearables provided by health care providers to monitor patients at high risk for adverse events, so they can intervene early and prevent emergency visits or hospital admissions.” 

Other people prefer keeping their own file of paper copies of medical tests and medications or computer copies. Regardless of which system you use, doctors say it is worth the effort.

Depression may be Confused with Dementia

Experts report as people age, dementia and depression are both common conditions and may be confused with one another. Additionally one may lead to the other as depression is linked to a higher risk for dementia, and dementia can cause depression. 

The Harvard Medical School explains depression in the elderly can lead to a “phenomenon called pseudodementia—an apparent intellectual decline that stems from a lack of energy or effort.” They say individuals with this problem are often forgetful, move slowly, exhibit low motivation and signs of slowing mental prowess. If a loved one experiences these types of symptoms, their doctor may treat it with the same medications used to treat depression. As the person’s mood improves, they typically begin to function at their previous level. 

Professionals share a few ways depression and dementia differs to help families and healthcare providers determine what is going on. One important difference is decline in mental functioning is typically faster in depression than with a person suffering from Alzheimer’s disease or another type of dementia. Alzheimer’s disease causes confusion and disorientation as well as problems with short-term memory. Depression causes problems with concentration. 

People suffering from depression do not have more than the usual difficulties with writing, speaking and motor skills, and are more aware of any memory problems they are experiencing. People suffering from Alzheimer’s disease seem indifferent to those types of changes. However, to determine the cause of the problem, it is important to make an appointment with the person’s healthcare provider.
